Electronic Health Record (EHR)

The Electronic Health Record (EHR) is a system designed to securely and efficiently store a patient’s complete health record in digital form. It enables access to essential information, including patients’ medical histories, test results, and other relevant details. The EHR module should capture and retrieve clinical information for each patient, serving as a central repository for all medical records. Data should be stored in a well-organized and structured format, allowing clinicians and healthcare providers to easily access the necessary clinical data based on their roles and requirements. The EHR will include the following functions and contents:

Demographic Information (patient) such as NID, Full Name, Father's Name, Mother's Name, Date of Birth, Nationality, Gender, Religion, Blood Group, Occupation, Contact and Emergency Contact Information e.g., Address, Mobile number, Email Address etc.
Health Details History such as Disease, Allergy, Surgical, Chronic disease.
Current Medical Info. such as Generic, Brand, Dose, Duration, Item Qty, Hospital, etc.
Past Medical Records such as Previous prescriptions, Previous Clinical Notes, Previous Lab Reports, Previous Findings, Previous Radiology and imaging reports, Previous Surgical History, Previous Hospitalization History, Previous Immunization History, and Previous Other related information.
Family Diseases such as Relationship with the Relative, Disease Name, Severity, Suffering, Suffering Duration, Death (if any)

This EHR with the above-mentioned features will be integrated and accessible through a shortcut key from the defined areas of the system. The following are the major areas:

EHR is a record of the patient’s mentioned information in a defined format. It will have a shortcut i.e., button which will have the ability to integrate in other areas as per requirement.
